What we commit to support coordinators.

01.

Response within one business day

You send a referral, we acknowledge inside 24 working hours. If we cannot take the placement, you hear that within 24 hours. No silent declines.

02.

Same-day intake one-sheet

On the day of the referral acknowledgement we send a one-page summary of the home being considered, the staff, the existing roommates if applicable, and our requested next step. PDF, email, or fax, your choice.

03.

Authorization renewals before they expire

We track your authorization windows internally. You receive a reminder from us 14 days before each authorization is set to expire, with the documentation you need to renew already attached.

04.

Quarterly resident updates

You hear from us quarterly about each person on your caseload who lives in our homes. Concise. Not a wall of text. Major changes get reported same-week, not at quarter-end.

05.

Honest population fit

If a referral is not the right fit for our current home composition, we tell you why and recommend providers who would be. No declines without an explanation.

06.

Salt Lake County only

We do not serve outside Salt Lake County. If you have a referral for another county, we are not the right answer. We will tell you that on the first call.

A short, honest list.

We do not pretend to be the right home for every person. Here is who we currently support best.

  • Adults (18+) on the Community Supports Waiver or another Utah HCBS waiver
  • Adults with autism, intellectual disability, Down syndrome, cerebral palsy, and combinations thereof
  • Adults whose support needs include medication administration, mobility assistance, and 24-hour staffing
  • Adults with a documented behavior support plan, including histories of behaviors that other providers have considered challenging
  • Adults who use augmentative and alternative communication, sign, picture exchange, or are nonverbal
  • Adults transitioning out of the family home, an ICF, or a previous provider

Where we are not currently the right fit: medically fragile placements requiring nursing-level care; adults under 18; placements requiring a single-person home (we are working on a single-person host home capacity for 2027). If you are not sure, send the referral anyway. We will tell you within a day.
